IN Memory of Tracey Anika Marcia Hosea Isokpan
Tracey Anika Marcia Hosea Isokpan was a devoted mother, sister, daughter, and lifelong learner whose life was defined by compassion, intelligence, and an unwavering commitment to those she loved. She carried herself with quiet strength and deep intention, leaving a lasting impression on everyone fortunate enough to know her.
To those closest to her, Tracey was a source of wisdom, encouragement, and care. She challenged others to grow while offering steady love and support. Highly educated and driven, she pursued excellence not only in her professional life but also in the way she showed up for her family and community. Above all, she was present, nurturing, and generous in spirit.
A Life Marked by Strength and Perseverance
Tracey’s health journey was largely unseen by many outside her immediate circle. She lived with Chronic Thromboembolic Pulmonary Hypertension, known as CTEPH, a rare and often underdiagnosed condition caused by persistent blood clots in the lungs. Although she underwent a medical procedure that initially appeared successful, her body continued to form clots, placing increasing strain on her heart and lungs.
Her immediate family, including her children, parents, and siblings, witnessed the reality behind the diagnosis. They saw the uncertainty, the resilience, and the determination it took for her to continue forward despite immense physical challenges.
Tracey’s passing revealed a difficult truth. Too many individuals and families face CTEPH without adequate awareness, timely diagnosis, or a full understanding of its long term risks.
Why This Initiative Exists
The Tracey Anika Memorial Initiative was created in response to that truth.
This initiative exists to honor Tracey’s life by transforming loss into purpose. By raising awareness of CTEPH, promoting education, and advocating for earlier diagnosis, we seek to help prevent other families from experiencing the same heartbreak.
Our work is rooted in remembrance, but it looks forward with intention. It is focused on education, advocacy, and meaningful impact for those affected by this condition.
Carrying Her Legacy Forward
Tracey believed deeply in knowledge, preparation, and doing things with care and intention. This initiative reflects those values. It exists not only to remember who she was, but to continue the work she would have supported, helping others, increasing understanding, and ensuring that lives are not lost simply because a condition went unrecognized.
Her legacy lives on through every conversation sparked, every individual educated, and every step taken toward better outcomes for those affected by CTEPH.
